It also depends on how well the mental health clinicians have documented mental health diagnoses and the severity of synonyms.

When you file a claim for Social Security disability benefits based on mental disorders, or if your mental disorder is part of your disability claim, whether you are applying for Social Security Disability Insurance (SSDI) or Supplemental Security Income (SSI), Social Security will review your medical records and determine if you have any severe mental impairment that impacts your ability to work. During the disability determination process, Social Security medical experts review your medical chart which should contain a clear indication of your diagnosis, progress notes created by your mental health clinician related to your mental health conditions, list of psychotropic medication you take if any, and your prognosis. Your mental health records or notes should also include the specific problems you are experiencing in your daily life, specific symptoms you exhibit, your progress in treatment, and any episodes of exacerbation or worsening. In addition, with some mental health conditions, there are objective tests that your mental health clinician can perform to support the severity of your mental health conditions.
